1. EXHIBIT ON DISASTER PREVENTION & MITIGATION and DISASTER RECOVERY & REHABILITATION

Inclusion:
- 12units collapsible/detachable backlighted wooden frame (1.54m x 0.20m x 0.90m) with
atleast two (2) T-light/frame
- 7units collapsible/detachable wooden Base (1.54m x 0.20m x 0.52m) with LED light
- 2units wooden post (.20m x .20m x 2.32m) for frame "L-shape" connector
- 1 unit LED WALL Frame (2m x 1m x 2.5m) with small door (for LED Wall), w/ cutout

Schedule:
Ingress: August 5, 2025 9:00AM - August 6, 2025 12:00NN
Event Proper: August 7-9, 2025 (3days)
Egress: August 9, 2025, 5:00PM


 Terms of Reference:
1. Supply and deliver ALL the exhibit materials indicated above at the Newton Plaza Hotel, Baguio City before the scheduled ingress on August 5, 2025.
2. Start plotting, unloading, hauling and installation of ALL exhibits materials at exactly 9:00AM of August 5, 2025 and finish the setup by August 6, 2025 at 12NN.
3. To provide at least two (2) skilled personnel at all times (1 installer, 1 LED technical) who will assist in the troubleshooting during the event proper on August 7-9, 2025, 3days event
4. Dismantle of ALL Exhibit Materials and other collaterals in good condition on the scheduled egress on August 9, 2025 at 5PM.
5. Return the exhibit fabricated at DOST-TAPI, Bicutan, Taguig City within 5day after the event.
6. Quotation is inclusive of VAT, Installation/dismantling, transportation and hauling services
7. Full payment shall be made after the completion of Service
8. Corresponding penalty will be imposed for the late setup and delivery on the scheduled time

Interested suppliers are required to submit the following valid and current eligibility documentary requirements (Note: Failure to submit the following documents will result to DISQUALIFICATION):

  1. Mayor’s/Business Permit (The line of business should be related to the procurement project).  In case not yet available, Mayor’s or Business Permit for the preceding year with the Official Receipt for renewal of application;
  2. PhilGEPS Registration Certificate/Registration No.; 
  3. Latest Filed Income Tax Return;
  4. Notarized or  Signed Omnibus Sworn Statement (OSS); and
  5. Duly signed Price Quotation Form (use Annex “A” ONLY).

Bidders must bid on ALL ITEMS. The contract will be awarded to the bidder offering the lowest quotation, provided it complies with the minimum technical specifications and/or description and falls within the ABC as stated above and along with other terms and conditions stated therein. Quotations exceeding the Approved Budget for the Contract (ABC) shall be automatically disqualified.

If the bidder “Passed” the Post-Qualification (PQ) and declared as Single Calculated Responsive Bid (SCRB) or Lowest Calculated and responsive Bid (LCRB), the bidder must submit the following applicable documents upon receipt of notification that they passed the PQ before the BAC issues the corresponding Notice of Award (NOA):

  1. For sole proprietorship - a notarized Special Power of Attorney, if a person other than the general manager or sole proprietor submits a bid for or represents the company for the bidding activities;
  2. For partnership, corporation, cooperative, or joint venture - notarized Secretary’s Certificate or Board/Partnership Resolution.

In the event that two or more bidders submit the same lowest bid, the Procuring Entity shall implement a non-discretionary and non-discriminatory tie-breaking procedure. Bidders are hereby informed that, in such cases, the Procuring Entity shall employ a method of chance, such as “draw lots” or any similar procedure, to determine the winning bidder.

Any interlineations, erasures or overwriting shall be valid only if they are signed or initiated by the bidder or his/her duly authorized representative/s. The Technology Application and Promotion Institute (TAPI) reserves the right to accept or reject any or all quotations and to impose additional terms and conditions as it may deem proper.
